How to Prepare for your Appointment

Please arrive 5-10 minutes before your appointment time to allow sufficient time to check-in and register.

WHAT DO I NEED TO BRING?

Any eye wear that you are currently wearing - this includes any driving, computer and reading glasses, sunglasses, full time wear glasses, safety glasses or sports eyewear
If you wear contact lenses please bring the box or a blister pack with your current brand and lens information
A list of your current medication
A valid Ontario Health card
If you have additional insurance, please bring your card or information with you
If you require completion of any forms or third party requests e.g. Ministry of Transport licence, employment or volunteer forms, please advise us ahead of your appointment

Late Cancellation and No Show Policy

If you are unable to attend your scheduled appointment, please notify the clinic at least 24 hours in advance otherwise you may be subject to a $75 fee.
